The clinical premise

Recovery is necessary. It is not the finish line.

Many physical therapy episodes begin after pain, injury, surgery, or a fall. Good care addresses that problem. A capacity-based model also asks what the person needs to return to, what future participation is at risk, and how much reserve exists beyond the minimum demands of daily life.

That wider lens creates a continuum: Recovery → Optimization → Preparation → Participation. The labels matter. Preparation avoids promising prevention; participation keeps the person’s life, not lifespan alone, as the outcome.

Geriatric Functional Milestones

A framework for noticing change.

The ten milestones were developed by Logan Taulbee, Ashleigh Trapuzzano, and Timothy Nguyen through the Brooks Rehabilitation Geriatric Residency and published in GeriNotes in 2021. They are observable tasks that can make function easier to discuss with patients, families, students, and referring clinicians.

Useful roles

Conversation opener, baseline description, patient-centered goal development, education, and a prompt for more formal assessment when change is reported or observed.

Important limits

The list is preliminary. It is not a validated scale, diagnostic tool, interval threshold, or substitute for standardized tests and clinical reasoning. Devices and adaptations should be interpreted in context.
